When investigating the root cause of failure, ideally the site of failure is assessed to obtain the necessary information to reconstruct the events backwards, from the observed damage and eyewitness recounts. Site documentation of the damage is, in most cases based on photographs taken on site. Fragments are usually good indicators of failure initiation and magnitude thereof, hence they are carefully collected for fractographic investigations and materials testing in the laboratory. The investigations are brought to an end by concluding on probable root causes through synchronization of all the test laboratory test results, site damage photographs and eyewitness recounts.
